The management of spinal metastases has become increasingly reliant on spinal stereotactic radiosurgery (SRS), a non-invasive treatment option that offers precise radiation delivery to tumors. However, the definition of target volumes in SRS remains a topic of ongoing debate among radiation oncologists and neurological surgeons. To address this issue, a recent study aimed to develop consensus target volume definitions for common scenarios in metastatic spine radiosurgery.

The study involved seven radiation oncologists and three neurological surgeons with spinal radiosurgery expertise who independently contoured target and critical normal structures for 10 cases representing various scenarios in metastatic spine SRS. The resulting contours were then analyzed using an expectation maximization algorithm for Simultaneous Truth and Performance Level Estimation (STAPLE) to calculate agreement between physicians.

The results of the study highlight the importance of achieving consensus on target volume definitions in SRS. According to the researchers, optimal consensus contours were identified using histogram agreement analysis and characterized to create target volume definition guidelines. The mean kappa agreement for gross tumor volume (GTV) was 0.65, while that for clinical target volume (CTV) was 0.64.

The study also found that the sensitivity and specificity of optimized confidence level consensus contours were high, with values ranging from 0.67 to 0.84 and 0.94 to 0.99, respectively. These findings suggest that the proposed consensus target volume definitions are both accurate and reliable.
